
Support in Your Own Home
We support people that are living in their own homes but may need some support to avoid them becoming homeless.
Adref has a team of staff in Rhondda Cynon Taf and Merthyr Tyfil that work with approximately 250 people per year. We draw up a support plan with the service user that states what areas the support will focus on; when we will meet; and for how long. The length of time for support can vary, though some people complete the support after 9 - 12 weeks. Once their support needs have been met the Supporting People team refers a new person. The support can encompass anything that helps people avoid homelessness.
These are just a few examples:-
- Identify the issues that have threatened a person’s housing
- Help people develop skills that will promote independence
- Ensure people are receiving the statutory service support and financial assistance they are entitled to
- Help people assimilate into their communities
- Re-establish relationship with family if appropriate
